The Individual Life Plan consists of the following four primary phases:

1. INTAKE PHASE

Abilis professionals capture relevant client information, including: diagnosis, medical history, family composition and history, specialist assessments, and educational history.

2. ASSESSMENT PHASE

Abilis professionals work with families to determine therapeutic and life skill needs, as well as expected outcomes. The Abilis team uses existing assessments or carries out new evaluations in the following areas: speech, occupational, physical, psychological and medical.

3. PLANNING PHASE

A team of Abilis specialists works collaboratively to determine the best course of action to achieve educational, social, employment, residential, and nutritional goals.

4. LEGAL AND FINANCIAL PLANNING PHASE

In conjunction with the ILP team attorney and financial planner, Abilis helps families understand and create documents such as a special needs trust to ensure that their families members with disabilities can qualify and receive needed government and community services.

The ILP Director consolidates all of the specialist input into the final Individual Life Plan, which provides a road map of resources, activities, and benchmarks for the client family to follow.
